Candidates must submit electronic records of all completed field experiences. These experiences may be instructional, classroom based experiences or non-instructional, training, or professional development experiences. Review the descriptions of each form below and select the most appropriate form for the experiences to be submitted. Review all tutorial videos and instructions on forms carefully to ensure accurate submission of data. Please ensure that the correct semester, course, and major are selected before clicking Submit.
  • Many field experiences have been submitted with incorrect program and course selections. In some instances, using the computer mouse's wheel to scroll will scroll to another option in a drop-down list, and that incorrect option is submitted. Please check your selections for each drop-down list on these forms before clicking Submit.

Initial Program (Undergraduate and M.A.T.) Field Experience Data Collection Form
All undergraduate and alternative certification (non-degree and M.A.T.) candidates use this form. Web-based form for recording field experiences electronically; Candidates must submit all data collected on the Field Experience Data Form (Self-Report) using this Web-based form.
 
Advanced Program (M.Ed. and Ed.S.) Field Experience Data Collection Form
All graduate M.Ed.--NOT M.A.T.--candidates use this form. Web-based form for recording field experiences electronically; Candidates in the Master of Education in Curriculum and Instruction (all concentrations) and Master of Education in Special Education must submit all data collected through field experiences here.
 
Professional Development Data Collection Form
Web-based form for reporting all candidates' professional development activities. Such activities are those field experiences that occur outside the traditional classroom setting (seminars, workshops, meetings, etc.).
 
Impact on P-12 Student Learning Data Collection Form
Web-based for for reporting P-12 student learning assessment activities completed during field experiences.
